Workers’ Compensation – Next Steps
I’ve had a workplace injury, now what?
Covered individuals may be entitled to apply for Workers’ Compensation benefits through the State of Washington Department of Labor and Industries (L&I).
Sought or Need to Seek Medical Treatment? – All Covered Individuals (Employees and Volunteers)
Step One
- Have your health care provider (HCP) complete a Report of Accident (ROA) at time of treatment. An L&I ROA must be completed before L&I can review and make a determination on a claim or associated costs.
- If you did not complete the ROA form at the time of medical treatment, complete a ROA via the L&I website or via the claims line at 1-877-561-FILE (3453).
- When you seek future medical treatment provide the HCP (Find an L&I Doctor) with the L&I claim number for billing purposes in order for the treatment to be covered by L&I insurance.
Step Two
- Have work limitations? Submit medical documentation/ updates to HRS Disability Services as soon as known.
Stay in contact with your L&I case manager during treatment.
L&I will notify HRS of a submitted claim and request any needed employer information.
Missed work associated with this event?
Step One
- Notify your supervisor of the need to take leave/be off of work.
Step Two – WSU Civil Service, Administrative Profession or Faculty only
- Request a Leave of Absence (LOA) through Workday. See Workday Materials Reference Guide Employee Request a Leave of Absence for instruction if you have missed three or more days.
- Complete and submit the Workday questionnaire.
- LOA requests received by HRS Disability Services will be assessed within five days for LOA provision eligibility.
- If eligible, the request will either be provisionally approved pending supporting medical documentation, if applicable, or will receive final approval if supporting documentation has already been received by Disability Services.
- If/when provisional or final approval is completed, LOA designations will appear in Workday Manage Absence Calendar.
Step Three
- Provide supporting medical documentation to WSU Disability Services supporting your LOA and return to work date including any limitations.
- IF HCP documentation, previously submitted to HRS, did not identify a specific date for full time return to work without limitation, have your HCP complete and return the Work Assessment form or an L&I Activity Prescription Form (APF) to HRS prior to your return to work
- Please note, that the option to have your HCP provide information on an APF versus another format is the employee’s choice. Washington Labor & Industries has indicated they may not pay for APFs completed at the request of employers or attorneys. Your HCP may provide the information on either linked form or in a format they choose.
