Complete the SmartHealth well-being assessment (worth 800 points) and then work on activities to reach 2,000 SmartHealth points by the deadline. If you do so, you can qualify for a $125 wellness incentive that will either reduce your 2025 deductible or be added to your Health Savings Account, depending on what plan you are enrolled in in 2025.
