Uniform Dental Plan
Uniform Dental is a preferred-provider organization (PPO) plan that allows you the freedom to choose any dentist, but provides a higher reimbursement for Washington dentists if your dentist contracts with Uniform Dental/Delta Dental of Washington. Out-of-state dentists are covered at the preferred rate, with the exception of slightly less coverage for preventative services. Participants pay a percentage of the cost (co-insurance) for dental services after the annual deductible has been met.
For additional information, contact Uniform Dental at (800) 537-3406 or visit HCA’s Dental Plans for PEBB page.
The plan offers services in every county of Washington State and worldwide.
Preferred Providers in Washington and dentists outside of Washington are covered under the preferred provider level, with slightly less coverage for preventative care for out-of-state providers. Dentists in Washington who do not elect to be preferred providers will have a lower level of coverage.
Contact Uniform Dental at (800) 537-3406 or use the online Provider Search.
Note: You must select the network Delta Dental PPO in your search to view the preferred providers covered in Washington under this plan. Again, all out-of-state dentists do not need to be contracted as preferred providers, but will have coverage at that level, with the exception of preventative care which will be slightly less.
Uniform Dental does not mail ID cards to participants. Please visit Uniform Dental Plan to register for an account and print your ID card or notify your dental provider that you are in Group #3000.
DeltaCare
DeltaCare is a managed-care plan, and it is recommended you select a DeltaCare network dentist when you enroll, or one will be assigned to you by the dental plan. You must receive services from a DeltaCare network provider; dentists outside the network will not be covered. Participants enrolled in this plan pay co-payments at the time of service. This plan does not have a deductible or annual maximum.
For additional information, contact DeltaCare at (800) 650-1583 or visit HCA’s Dental Plans for PEBB page.
DeltaCare Certificate of Coverage
You can elect DeltaCare regardless of where you reside, but will need to go to where the DeltaCare Dentist is located. Note, there are no DeltaCare dental offices in the Pullman/Moscow area, and the overall DeltaCare network is small.
Contact DeltaCare at (800) 650-1583 or use the online Provider Search.
Note: You must select the network DeltaCare in your search to view the providers covered under this plan. Please verify your dentist contracts with DeltaCare as Delta Dental of Washington administers several dental plans, each with its own provider network. This is important, as you could be responsible for costs if you receive care from a provider who is not in the DeltaCare network.
DeltaCare does mail ID cards to participants. You may also notify your dental provider that you are in Group #3100.
Willamette Dental
It is a managed-care plan, and it is recommended you select a Willamette Dental network dentist when you enroll, or one will be assigned to you by the dental plan. You must receive services from Willamette Dental dentists or specialists; dentists outside the network will not be covered. Participants enrolled in this plan pay co-payments at the time of service. This plan does not have a deductible or annual maximum.
For additional information or to find a provider contact Willamette Dental at 1-855-433-6825 or visit Willamette Dental Plan for PEBB.
Download the Summary of Benefits and Certificate of Coverage from the Willammette Dental website.
You can elect Willamette regardless of where you reside, but will need to go to where the Willamette Dentist is located to have coverage. Note, there is only one Willamette dental office in Pullman, and the overall Willamette network is small.
Call 1-855-433-6825 or use the online Wilammette Provider Search.
Cards are not necessary as your provider will have your plan information.
